The company maintains its principal operations through American Healthcare REIT Holdings, LP, its operating partnership. The company's business model centers on generating current income and capital appreciation through strategic real estate investments while maintaining financial flexibility and making regular cash distributions to stockholders. American Healthcare REIT operates through four reportable business segments: Integrated Senior Health Campuses (ISHC), Outpatient Medical (OM), Senior Housing Operating Properties (SHOP), and triple-net leased properties. As of December 31, 2025, the company owned and operated 147 ISHC facilities, 74 OM buildings, 97 senior housing and skilled nursing facilities in its SHOP segment, and six SNFs plus eleven senior housing facilities and two hospitals within its triple-net leased properties segment.
The ISHC segment provides a continuum of care including independent living, assisted living, memory care and skilled nursing services within single campus settings, predominantly operated under RIDEA structures that allow the company to participate in operational upside while bearing operational risks. The OM segment consists of multi-tenant medical office buildings leased to healthcare providers under long-term leases with expense recovery provisions. The SHOP segment operates senior housing and skilled nursing facilities serving elderly populations with varying care needs. Triple-net leased properties generate stable income through tenant-paid operating expenses and capital expenditures.
American Healthcare REIT maintains a fully-integrated management platform with approximately 121 employees as of December 31, 2025. The company has elected to be taxed as a REIT for U.S. federal income tax purposes and distributes at least 90 percent of REIT taxable income to stockholders annually. The company may invest opportunistically in real estate-related investments including mortgage loans, mezzanine loans and bridge loans on an infrequent basis. American Healthcare REIT pursues growth through disciplined acquisitions, selective development partnerships, organic growth from its long-term care portfolio and active balance sheet positioning.
