SynCardia's core product, the SynCardia TAH, is a biventricular replacement device that fully replaces the function of a failing human heart by replacing both ventricles and all four heart valves. The device consists of an implant with left and right artificial ventricles, external pneumatic drivers that power the implant, and drivelines connecting the external driver to the implant. The system generates pulsatile flow with cardiac output up to 10.5 liters per minute for the 70cc implant and 7.5 liters per minute for the 50cc implant. To date, more than 2,100 SynCardia TAHs have been implanted across 27 countries, establishing the device as an established bridge to heart transplantation for patients with end-stage biventricular heart failure.
The SynCardia TAH received FDA premarket approval in 2004 and Health Canada approval in 2005 for use as a bridge to transplantation. The company offers two external driver systems: the Companion 2 Driver for in-hospital use, approved by the FDA in 2012, and the Freedom Driver for portable home use, approved in 2014. Both drivers connect to the implant via drivelines and deliver precisely calibrated air pulses to operate the artificial ventricles.
The company completed its initial public offering in September 2025, raising approximately $15.2 million in net proceeds. Picard Medical is pursuing multiple growth initiatives including development of the Emperor TAH, a next-generation fully implantable artificial heart designed to eliminate external pneumatic drivers, with potential FDA approval targeted as early as 2028. The company is also developing the Unicorn driver system to improve portability and patient quality of life, and seeking expanded indications for the SynCardia TAH including bridge to candidacy and longer-term support.
