Vivani's core business focuses on designing and developing drug implant candidates engineered to enable reversible, ultra long-acting, near constant-rate delivery of therapeutic medicines for treating chronic diseases. The company addresses a significant healthcare challenge: approximately fifty percent of patients do not take their prescribed medications as directed, whether daily oral or weekly injectable formulations. This medication non-adherence contributes to poor clinical outcomes and substantial healthcare costs.

The company's lead programs target metabolic diseases. NPM-139 is a semaglutide implant candidate in development for chronic weight management in obese and overweight patients, anticipated to enter clinical testing in mid-2026. NPM-133, also a semaglutide implant, targets type 2 diabetes treatment. NPM-115 is an exenatide implant for chronic weight management that completed its first-in-human clinical trial, LIBERATE-1, in 2025, demonstrating positive safety and tolerability profiles. Vivani's NanoPortal technology utilizes a biocompatible titanium-oxide nano-porous membrane with precisely sized nanotubes enabling near constant drug release without moving parts. The implants are designed to deliver therapeutic drug levels for six months or longer with convenient once or twice yearly administration while maintaining the ability to be removed at any time.

The company resulted from the August 2022 merger of Second Sight Medical Products and Nano Precision Medical. Vivani subsequently contributed Second Sight's neurostimulation assets to Cortigent, a wholly owned subsidiary, which is pursuing a separate public listing through either an initial public offering or spin-off transaction.

Vivani operates manufacturing facilities in Alameda and San Diego, California, and maintains a subsidiary in Australia supporting clinical development activities. The company competes within the GLP-1 receptor agonist market, which generated approximately seventy-two billion dollars in combined global sales across obesity and type 2 diabetes indications in 2025.
